change of name
my pangga and i remembered yesterday morning that i will need to change my surname when we get back this august.. so there's the passport, my sss card, bank account.. does anyone have a complete checklist for the change of name? =D do i need to do it back home or can i do it from SG? im not sure ill be able to do everything in the time im in PH.. =S

ang hassle noh? you only need to change sa password pag ire-renew mo na.
do you need your SSS pa ba? lol. kung hindi na, wag mo na papalitan, oks lng yun hehe.
depende kung san ka magpapapalit ng name.. kung sa bank, ask them baka pwede magfax ka na lng ng CTC ng MC nyo.. kung sa government, good luck na lng. l o l.

passport, sss, driver's license, bank accounts, office records, philhealth, medicare, tax return record (you might want to file jointly). and it's actually a change in marital status, the change of name just goes with it. hehehe!
ikaw na mag-file, then bigyan mo na lang ng POA or any kind of authorization kung sino man magfa-follow-up. and leave an ID, minsan kailangan nila yung ID pati nung nag-authorize. hope those info helps! if you need more info, tanong mo si mudra. hehehe!
